Class or Category I

• Recent chest pain or angina equivalent in an intermediate or high-risk patient, scheduled for intermediate or high-risk surgery

Class II indication

• Diabetes mellitus patients
  • patient with prior revascularization
  • asymptomatic male older than 45 years
  • asymptomatic female older than 55 years with two or more cardiac risk factors

Class or Category III

• Routine test in asymptomatic patients undergoing low-risk procedures
